Telemonitoring Services
Qualified patients are provided with cellular-enabled monitors. Once a reading is taken, the monitor will automatically upload it to our HIPAA compliant online portal. Physicians have 24/7 access to patient vitals.

Eligibility
We will ensure insurance coverage for the telemonitoring program as well as qualifications for necessary risk factors.

Contact
Once eligibility is confirmed, patients are contacted about the program and its benefits

Delivery
Patients who elect for telemonitoring services will be provided a new monitor by one of our nurses. We always make sure to hand-deliver and teach proper usage of equipment.

You're All Set
Patients are set to start taking vitals from day one. Chronic conditions are managed and monitored 24/7.
